HEALTH EQUITY IMPROVEMENT PROJECT

Project Name: Health Equity Improvement Project

Project Phase: Design

Brief Summary: This improvement project will work toward reducing or eliminating disparities in outcomes based on race, ethnicity, and insurance type by identifying and removing barriers to prenatal care, including prenatal diagnosis and coordinated delivery of mothers of infants with congenital heart disease.

SMART Aim: By December 2023 we aim to:

  • Decrease the gap in rates of coordinated delivery by 50%:

    • Between White and Hispanic patients

    • Between White and Black patients

    • Between White and “Patients Who Identified as Other”

  • Decrease the gap in rates of prenatal support provided between commercial insurance and government insurance from 5% to 0%

Participation Details: NPC-QIC centers are invited to participate in the ongoing monthly Health Equity webinars. Announcements for webinar dates and links to register are sent out via the Quality Times Newsletter and SharePoint site. At the conclusion of the webinar series, centers will be invited to continue their participation through the use of QI tools to reliably test health equity interventions.
